Insurance
CFA participates in most major insurance plans. Please contact our Billing Department at (860) 437-4550 regarding participating insurance.
Client Financial Responsibility
You are ultimately responsible for the payment of your treatment and care. It is your responsibility to know your benefits under your health plan. Please understand that the financial responsibility for services is between you and your health plan. Your insurance policy is a contract between you and your insurance company.
CFA will bill your insurance company as a courtesy; however, you are responsible for providing us with the most recent and accurate insurance information. You will be responsible for any charges incurred if the information provided is not correct or up to date. It may also become necessary for you to contact your insurance company to provide them additional information in order for them to process your claim completely.
For all services except the School-Based Health Center program, you are also responsible for required co-payments, co-insurance, applicable deductibles, and any services that are not covered by your insurance plan. You are responsible for paying any co-payment at the time of service. For School-Based Health Clinic services, health insurance is billed for services provided, but there is no out-of-pocket cost to the family (no co-pays, deductible costs, or out-of-pocket expenses).

Financial Support
CFA can assist you in meeting your financial obligations:
CFA offers a sliding fee program for clients who are uninsured or underinsured and whose income is at or below 200% of the federal poverty level. CFA’s sliding fee scale is based upon a client’s family size and household income. Clients who do not provide income information are responsible for services at CFA’s full rate.
Patients may need to provide the following information when applying for a sliding fee scale: Copies of tax returns, pay stubs, or other information verifying income may be required before a discount is approved.
The payment plan will allow you to make weekly, biweekly, or monthly payments on your account until the entire outstanding balance is paid. The payment amount and number of payments will be decided during your financial consultation with the CFA Billing Department.
